The Copyright Office application forms are available in the Adobe Acrobat PDF format. You must have the latest Adobe Acrobat Reader installed on your computer to view and print the forms. The Adobe Acrobat Reader is available for free from Adobe Systems Incorporated.Important Printing Instructions: The forms should be printed head to head (top of page 2 is directly behind the top of page 1) using BOTH SIDES of a single sheet of paper. NOTE: Short forms are one sided. Dot matrix printer copies of the forms are not acceptable. Inkjet printer copies of the forms require enlarging if you use the Shrink to Fit Page option. To achieve the best quality copies of the application forms, it is recommended that you use a Laser Printer.
The forms submitted to the Copyright Office must be clear, legible, and on a good grade of 8.5-inch by 11-inch white paper. The quality of the copyright application forms submitted for registration directly affects the quality of the copyright registration certificate you receive. The Copyright Office produces completed registration certificates through an optical scanning system that utilizes an image scanned from the original application submitted. As a result copyright applications submitted that are of poor print quality will negatively affect the print quality of the copyright registration certificate you receive. For these reasons, the utmost effort should be made to submit the best quality application form possible.

Fill-in Forms: Copyright Office fill-in forms allow you to enter information while the form is displayed by an Adobe Acrobat Reader product and then print the completed form out. Fill-in forms give you a cleaner sharper printout for your records and for filing with the U.S. Copyright Office.

- Form GR/CP Non Fill-in version - An adjunct application to be used for a group of contributions to periodicals in addition to an application Form TX, PA, VA.
Format: Notice of Intent - Format for filing a Notice of Intent to Enforce a Copyright Restored under the Uruguay Round Agreements Act (URAA). For further information see Restoration of Certain Berne and WTO Works.
Statement of Account Short and Long Forms- Licensing Forms SA1-2 Short and SA3 Long are now available in non fill-in versions.
- SA1-2 Short Form Non Fill-in version - For use by cable television systems whose semi-annual gross receipts are less than $292,000.
- SA3 Long Form Non Fill-in version - For use by cable television systems whose semi-annual gross receipts are $292,000 or more.
